This form is for all your data regarding what species you are hearing and temperature, weather, etc. It is meant to cover all the months of the survey unless you make more than 31 observations. Use a second form if you survey more than that. This form is due at the end of the survey after July 31; please get it in by August 15. Please also send in your area map with marks on it for your actual survey sites.
Volunteer Tracking Form

This form is to track ALL of the time that you dedicate to the survey. This should include anything you do as part of the survey, including: reviewing training materials and calls, checking out your site before the survey begins, travel time, time spent filling out forms, and of course the time spent surveying. This form must be filled out monthly and signed by all surveyors. Since we need actual signatures, it must be mailed or faxed in. Please send this in the first week of the month following each survey month.
